Leading the Mission: Dina Fine on Building TailorMed's R&D Organization

Dina Fine has always liked difficult problems. Math came naturally to her, and engineering gave her a way to turn complex problems into products people use.
"I really love solving problems and building things," she said.
As Vice President of Research and Development, Dina leads, builds and hires the engineering organization behind TailorMed. She joined nearly six years ago, when the company and its technology looked very different, and has shaped how R&D architects, builds and ships ever since.
The problem her team is solving
TailorMed is the medication success platform, built so that every patient who is prescribed a medication can get it, afford it and stay on it. The network behind it reaches 75M+ patients and 335M+ prescriptions across 950+ hospitals, 4,700+ clinics and 3,100+ pharmacies.
At that scale, it is a data problem first. Clinical, financial, pharmacy and manufacturer data arrive fragmented, in different formats and on different schedules. Dina's team streams and processes that data continuously, normalizes it and turns it into a real-time view the platform and its AI can reason over and act on.
The data is sensitive and the decisions matter, so security, privacy, accuracy and performance are designed into the architecture. "Our customers and patients need to trust what they see in TailorMed," Dina said.
How Dina runs R&D
Dina has built an AI-native engineering organization. Engineers use AI across the development lifecycle, from exploring solutions and writing code to testing, reviewing and debugging, and the team constantly experiments with new tools and workflows.
"The speed is crazy. What we can build today, and how quickly we can build it, is something I couldn't have imagined even a year or two ago," she said.
Her rule for that speed is clear: AI-generated code is code the team owns. "AI can help us move incredibly fast, but our engineers are still responsible for what we deliver." Engineers answer for the architecture, performance, security and quality of everything they put into production.
She structures R&D around product domains, each owned by an engineering leader with real independence, and deliberately keeps those domains from becoming walls. When one team needs help, people step in and resources move. "We win together and we deliver together," she said.
What's next: agentic systems
Dina's next challenge for the team is agentic AI that runs complex medication workflows end to end, not one more automated task. Across coverage verification, prior authorization, program enrollment and adherence, TailorMed is building systems that understand context, reason over data, decide what happens next and take action.
The hard problems are agent orchestration, reasoning over incomplete data, accuracy, observability and control, in a domain where mistakes matter. "Our job is to make the experience much simpler for patients and care teams," Dina said.
Who Dina hires
Dina's bar starts with technical excellence. She looks for engineers who:
• Enjoy technically hard problems and understand deeply how systems behave
• Want to work with scale, data and AI, and keep learning new technologies
• Design solutions rather than implement specifications
• Take ownership: understand the business need, challenge assumptions and make technical decisions
Her advice to engineers who want to grow: understand why something needs to be built before deciding how, and "take ownership before somebody gives you the title."
Outside of work, Dina reads, bakes and travels in Italy. She is also learning Italian, simply because she enjoys it and likes having a beginner's challenge again.
